CEDPA, a nonprofit international NGO, is under contract to USAID to provide Technical Advisors in AIDS, Child Survival and Basic Education (TAACS) to USAID offices overseas and in Washington.
The specialist, a member of the office management team, is responsible for providing support for a complex range of HIV/AIDS programme operations, including strategic budgeting and budget formulation, activity design and approval, procurement planning and execution, and budget control and administration. The specialist will contribute to OHA's progress toward the Bureau for Global Health's strategic objective to increase the use of improved, effective, and sustainable responses to reduce HIV transmission and to mitigate the impact of the HIV/AIDS pandemic (SO 4). The candidate will be a member of the office management team and report to the Deputy Director.

RESPONSIBILITIES:

• Assist in the formulation, administration, and analysis of all OHA budget and procurement planning processes and documents necessary for effective implementation of and reporting on HIV/AIDS resources
• Assist in the strategic planning for GH/OHA, including working on the annual documents (i.e., CBJ, BPBS, APS, etc.) related to the GH Bureau's strategic plan. Participate in the defining and refining of programme results, participating in the planning exercises of the strategic objective teams, and translate these exercises into budgetary request levels for the office
• Work collaboratively with the office management team and OHA staff to design, procure, and fund services necessary for the expansion and scale-up of the HIV/AIDS activity portfolio, following USAID policies and procedures.
• Provide support and guidance to OHA staff regarding Agency-level requests, decision making, and issue resolutions around procurement, budgeting, and planning activities.
• Provide budget analyses throughout the fiscal year to prepare leadership for setting office OYB and agency HIV/AIDS assistance funding levels. Prepare reports/presentations relating to GH/OHA and agency-wide funding levels and programme activities.
• Provide technical assistance to, and maintain close relationships with, regional bureaus, PPC/B, M/OP, field missions and field support team with regard to HIV/AIDS budget and procurement related issues.

REQUIREMENTS:

• Must be a US citizen eligible for secret clearance investigation
• The candidate should have a master's degree or equivalent work experience and a minimum of 10 years' experience in international development
• Demonstrated experience in financial management and procurement, including a mix of field and headquarters is preferred
• The ideal candidate will have an in-depth understanding of US government budgeting and procurement policies and procedures, and have some professional familiarity with the public health sector.
• Recent experience with the design, budgeting, and/or procurement of HIV/AIDS programmes is desirable

The USAID's assistance to India for a comprehensive development programme has an annual portfolio of over US $100 million dollars. Of this, nearly $50 million is programmed in health. USAID/India's health strategy and activities play a central role in achieving the Agency's worldwide objectives to improve reproductive health, stabilise world population growth, and prevent and control the spread of HIV/AIDS. The Population, Health, and Nutrition (PHN) Office is comprised of one strategic objective (SO) which addresses reproductive health, child health, and infectious diseases.

India is second only to South Africa in total number of HIV infections: there are 4.5 million HIV positive people in India. The epidemic has been focused in the past and the India National AIDS Control Organisation has identified six Indian states as high prevalence states for HIV. However, the epidemic is spreading to other parts of the country. USAID has been assigned by the Government of India (GOI) to work in two of the high-prevalence states, but USAID has HIV activities in other parts of the country.

The current HIV/ID portfolio in India is comprised of activities in HIV/AIDS prevention and control, polio eradication, tuberculosis control, applied epidemiological research and support for surveillance system development. The TAACS scope of work would not be limited to only HIV/AIDS.

RESPONSIBILITIES:

• The incumbent will be expected to play a leading role in USAID/India's HIV activities. S/he will be a technical expert in the rollout of the newly approved HIV strategy and be a key part of the team, implementing a comprehensive response to HIV/AIDS in India. S/he will also be a key actor in the management of other infectious disease activities, particularly tuberculosis
• Evaluate, modify, and scale-up the implementation of the Mission's new HIV strategy approved in December 2003; look at innovative approaches to increase activities in care and support
• Provide technical expertise in the design, implementation and management of the Mission's HIV strategy
• Provide analysis and advise the Mission on HIV/AIDS and TB sector policy and reform programmes, and track the USAID HIV and TB programme implementation, performance and results
• Help develop strategies, results frameworks and performance monitoring plans for the HIV/ID Team and promote cross-linkages to other SO teams as appropriate
• Be responsible for preparing documentation related to the portfolio and contribute generally and specifically to the Mission Management Review, R4s, the Mission Performance Plan and other documentation as appropriate
• Provide technical assistance to the Infectious Diseases activities, particularly in HIV/TB

REQUIREMENTS:

• PhD or MD in health, public health, epidemiology, management, sociology, or related social science field preferred. MPH or MSH will be considered based on experience
• Minimum of five years' experience with bilateral and centrally funded health programmes and demonstrated broad technical knowledge and experience in health analysis and planning, and planning and management of programmes in developing countries.
• Demonstrated leadership, analytical and managerial skills within the context of developing countries
• Knowledge of other relevant donor programmes and procedures in Asia
• Candidate should have prior experience managing HIV programmes
• Demonstrated writing skills (report writing)
• Basic computer skills: Word Perfect, Excel, PowerPoint.

TECHNICAL KNOWLEDGE:

• Conceptualisation, design, management and coordination of USAID-financed activities related to HIV/AIDS prevention, care and treatment
• Provision of technical advice and recommendations in planning, designing and developing activities in health and/or HIV, as part of a mission strategy or other donor-relevant activities and programmes
• Application of sociological, anthropological or related cultural knowledge in addressing issues in the design, implementation and/or evaluation of a development-related programme/projects
• Analysis and monitoring of complex health situations

Tanzania is two decades into a severe AIDS epidemic that threatens to disrupt the economic and social development gains of the past thirty years. Eleven percent of blood donors are infected, according to the National AIDS Control Programme's most recent surveillance report. Among antenatal clinic attendees, the infection rate is calculated at 9.6 percent. Although AIDS cases are chronically under-reported, approximately 60 percent of urban hospital bed occupancy is attributed to AIDS. Tuberculosis is increasing as a direct result of AIDS, in a country whose National Tuberculosis and Leprosy Programme was achieving treatment success rates of 80 percent in the early 1980s; the number of TB cases has risen from 11,000 in 1983 to over 60,000 in 2000. TB is the leading cause of mortality among AIDS patients, accounting for 30 percent of all deaths. Finally, AIDS is having devastating impact on Tanzania's children: over 1 million children have been orphaned by AIDS.

With a National Care and Treatment Plan now in place, rapid expansion of care and treatment programmes, including the prevention of mother-to-child transmission, is beginning to scale up. VCT is in an expansion phase and a successful social marketing programme has created a new challenge, with demand for VCT services outweighing current service supply. IEC and BCC programmes have achieved a certain measure of success: 96 percent of Tanzanians are aware of AIDS. However, this high level of awareness has not automatically translated into positive behavior change in terms of condom use or other preventive measures.

Under the new mission strategy, a previously integrated health and HIV/AIDS SO is being divided into two objectives with separate team leaders. While integrated activities will exist between HIV/AIDS and Health, the HIV/AIDS team is evolving, given the increasing demands of PEPFAR. While the exact make-up of the team is still being determined, at a minimum it will consist of a five-person core team, including the Team Leader, the TAACS HIV/AIDS Advisor, two Technical FSNs and one administrative assistant. A Behavior Change Specialist and a Policy Advisor are also integral members of the HIV/AIDS Team. In addition to the core team, the Health Team and other USAID-funded partners in both the public and private sector make up an expanded HIV/AIDS team, working together to achieve the strategic objective.

The TAACS HIV/AIDS Advisor for Care, Treatment and Support will take a leadership role in ensuring the USAID programme, under PEPFAR, supports the goal to ensure the establishment, development, and implementation of HIV treatment programme that includes management of HIV, using antiretrovirals and the management of opportunistic infections and cancers. The advisor will oversee expansion of the antiretrovial programme and ensure that it is part of a continuum of care, from VCT, PMTCT, STI management to palliative and home-based care. S/he will lead the Continuum of Care sub-group on the USAID HIV/AIDS team, and ensure that USAID's programming efforts in antiretroviral management, antiretroviral drugs and other commodity logistics, palliative and home-based care, VCT, PMTCT, and other related programming, are coordinated to achieve the targets set under PEPFAR. This includes ensuring that USAID's activities are coordinated with other SOs in the Mission and with the Government of Tanzania, other donor partners, and the efforts of US government agencies such as CDC, HRSA, DOD, Peace Corps, and their implementing partners.

RESPONSIBILITIES:

• Provide technical leadership for the design and implementation of Care, Support and Treatment initiatives under Mission and PEPFAR activities
• Lead HIV/AIDS sub-team on "Continuum of Care" to ensure a cohesive and coordinated USAID mission approach to supporting Continuum of Care partnerships under PEPFAR that contribute to reaching USG goals under PEPFAR
• Liase with CDC, DOD, Peace Corps in coordinating, planning and implementing USAID activities related to ensuring a functioning Continuum of Care at district level and below
• Perform CTO duties for identified partners under USAID Antiretroviral treatment, Care, and Support programming
• Remain current in the developments in the field of HIV management, including antiretroviral treatment, Care and Support, VCT, PMTCT
• As an HIV/AIDS team member, provide technical support to the mission's HIV/AIDS strategy and ensure appropriate linkages with mission Health strategy and other objectives
• Liaise with other relevant donor, non-government, and public sector entities and programmes as appropriate
• Ensure/coordinate appropriate design and implementation of monitoring and evaluation plan for continuum of care partners under PEPFAR
• Prepare required documentation related to mission management reviews, including portfolio reviews, Annual Reports, Mission Performance Plan, and other documentation as appropriate
• Supervise FSNs within HIV/AIDS team

REQUIREMENTS:

• MD or PhD in public health, health management, community care, clinical care or equivalent. Postgraduate training in HIV medicine will be an asset. MSN, MPH, or MSH will be considered, based on experience
• Minimum seven years' experience in managing bilateral or centrally funded health programmes and demonstrated broad technical knowledge and experience in analysis, planning and management of health programmes
• Demonstrated experience in leading vision building and results-focused implementation among multiple partners
• Demonstrated ability to effectively multitask and meet deadlines in a high performance team environment
• Demonstrated writing skills (report and other key communications)
• Basic computer skills, including Microsoft Word, Excel, PowerPoint and Outlook
• Strong interpersonal skills and demonstrated ability to be a team player

3. Application of knowledge of indigenous behavioral social support issues and their relationship to programme design, implementation and evaluation.
